 Please make sure that all listed items are provided at the time of application.

•      Enrollment agreement

•      Emergency procedure card for each child.

•      Release of student record form

•      Health appraisal filled completely by physician and immunization record

•      Birth Certificate

•      Copy of report card or transcript for middle and high school students

•      Any other important records such as IEP or ESL
